Apply thickness

I'm having a problem with Apply Thickness (2013 sp1). If I apply it to a cylinder, or tube shape, it doesn't work. I looks like it's merging all the points of the thickness into a single point in the middle of the mesh.

If I apply it to a flat grid, or a grid where I've moved points via prop modelling tool, it works as expected. If I try to apply Fold to shape the grid into a cylinder, it again fails to work.
