Like wrote in the object, my Local Manipulator wrong position.
Sometime really little error:
Other more exaggerated:
Why? Sometime the manipulator handle are totally distort in strange position. Do local mode position handle respect normal object or not?

Local means with respect to the current selection, whether it's objects, points, polygons, etc. Try the other modes -- I'm not 100% sure but judging from your image I think that maybe you want Object mode.

Thats because you have non-uniform scaling applied to the object. Try freezing the scaling.

Sorry, that wasn't what I meant. Select the item as an object (it's currently set to polygon) and set the coordinate mode back to local again.
Hint: the selected item (wire) turns white when you are in Object selection mode

Then there is the possibility that you may have had reset all transformations at some point. This means if you turn your mesh 45degrees on whatever axis, and then freeze all transformations, the angle will now be 0 degree and not 45 like before. Because of this the handles will point in "wrong" directions.
You can fix this by manually aligning/rotating the "Center" of the object.
